Biotechnology is defining the future of rare disease care, driving the most promising developments from advanced therapies to precision approaches. The Biotech Act presents the opportunity to bridge innovation with industrial competitiveness strengthening its position as a destination for rare disease research. While shorter clinical trials will be key to ensure faster time-to-market, regulatory sandboxes reduce uncertainty and support emerging technologies. This needs to be backed by a credible investment signal, with predictable funding that makes rare disease development viable, de-risks innovation and strengthens trial capacity and data infrastructure, particularly for SMEs and early-stage innovators. Europe’s ambition is clear, the critical point is whether the Biotech Act, and its forthcoming second phase, will match the level of coordination, flexibility and investment needed to deliver.

No One Behind: Aligning Access and Sustainability in Rare Disease Care

For rare disease patients, where they live in Europe still too often determines whether they are diagnosed in time and can access appropriate care. Even after EU-level approval, access to early diagnosis, treatments and specialised services varies across and within Member State and regions, shaped by fragmented reimbursement systems, uneven budget capacity and divergent implementation. These inequalities have consequences beyond health outcomes, leading to avoidable disease progression and long-term disability, and creating a wider societal and economic burden for families, caregivers, workplaces and health systems. Rare autoimmune diseases provide a clear illustration of these dynamics, showing how differences in delivery translate into real-world access and broader societal impact. This entails a structural trade-off: reducing inequalities in access while maintaining financing models capable of supporting resilient systems and ongoing innovation.

From Global to Local: Action & Governance in Rare Diseases

Turning rare disease policy into impact depends on governance structures that connect European, national and increasingly global frameworks into consistent delivery across regions. While the European Reference Networks have strengthened expertise-sharing and cross-border care, their long-term sustainability and integration into national systems remain uneven. Implementation still varies widely across Member States, particularly in referral pathways to centres of expertise, access to diagnostics and treatments, data collection and registries, and the resourcing of specialised services, reflecting coordination gaps and variable capacity. As discussions advance on the next EU Multi-annual Financial Framework, and following the World Health Assembly resolution recognising rare diseases as a global public health priority, aligning EU instruments with national plans and international commitments becomes central, strengthening accountability and securing sustainable funding so that best practices translate into measurable impact for patients across Europe.

Capitals in Dialogue: Sharing Practice, Shaping Policy

A better EU ecosystem for patients will ultimately depend on its political prioritisation at Member States. In this sense, the rare disease policy landscape in Europe reflects wide differences in political ambition, implementation and capacity. Where successful practices exist, they offer evidence and base to work on; where gaps persist, they point to the need to rethink the system and open the window for reflection. In the field of rare diseases, exchange between countries is particularly valuable: expertise is limited, patient populations are small, and many health systems face similar bottlenecks in diagnosis, care coordination and access.Better exchange between countries, combined with stronger links between national and European efforts, will be critical to transfer successful approaches across pan-European contexts.
